If I ve signed a consent form for a tubal ligation but I move our of state and get a new doc, do I need to sign a new one?

I am 30 weeks pregnant. I m on Medicaid and currently live in Kansas. I ve signed a consent form from my doctor to get a tubal ligation done after I give birth. I am relocating to Florida in a month. When I find a new doc in Florida do I need to sign a new consent form or does the first one I signed from my doctor in Kansas still count. Unfortunately I m too far in my pregnancy that if I do need to sign a. New one I don t think the 30 day waiting period will pass before I give birth.

You do need to contact your caseworker since you are moving to another state prior to delivery. Medicaid will have to transfer to the new state. Do you already have a doctor set up there? Has Medicaid been OK'd for Florida?



If not - you could be stuck with no doctor who wants a 34 week pregnant patient with no insurance.



You just cannot show up in Florida and have the local OB to right away accept you so late in pregnancy. You need to get this set up now before you move.



Same for transferring your WIC case too. Call and get the ball rolling. Do not wait till you get to Florida with no doctor and no food assistance.

You will likely need to sign a new one, since a waiver is for a specific doctor, procedure, and facility.
